Context rules this.

The term "homo" is tossed around in Socal if someone is just being dumb, as slang perhaps, an idiom. But, go to SanFrancisco and call out "homo!" to someone who is one, it could be taken as derogatory.

It's not the words themselves, it is how they are used.

Southern poltician referring to a mature man as "boy" is an attempt to demean his credibility in maturity.

Southern politician referring to a black man, who is another politician as "boy", is a pretty stupid slip. It has a connotation to it.

Who knows why he said it. But he did, and he shouldn't apologize for it, if he feels it is appropriate to call Obama "boy".
